This form is for Certified LeadYoung & LeadWell Trainers only, using LeadWell curriculum (ages 14-18). (If you're ordering LeadNow & LeadWell materials, complete the different forms, but let us know of the 2 orders so we can ship them together.)

Trainers, please fill out this form and submit it to KidLead, at least 10 business days prior to your start date, so that we can effectively get the materials to you in time. If you have any questions, don't hesitate to call or e-mail.  We appreciate your investment in young leader development.

1st Module Students/Leaders: first & last name, application date, #1 SIS score, #2 SIS score (i.e. Jesse Nelson, AM, 5/13/11, 3.88, 4.04).  Please start a new line for each participant.  These leaders will receive regular handouts, concept cards, and completion pin, along with 1st time materials: LeadYoung book & Training Manual.

 

2nd Module Students/Leaders: They'll receive a LeadWell portfolio & pen along with their regular handouts, completion pin and concept cards. 

 3rd Module Student/Leaders: They'll receive a v-neck, LeadWell t-shirt, along with their regular handouts, completion pin, and concept cards.  Be sure to clarify the t-shirt size and whether it is male or female.  Sizes include Adult Sm, Med, Lg, XL, XXL, XXXL.

 

4th Module Student/Leaders: They'll receive a 6" chenille LeadWell letter "L" and a framed certificate, so please be sure spelling is proofed.  This is in addition to the regular handouts, completion pin, and concept cards.

 

Koach cues (How many sets do you need?)  

 Misc.  Please provide us with any special instructions or re-orders for books, name badges, notebooks, Koach shirts, etc.  We'll let you know if there are any extra charges before shipping the order (i.e polo shirt, $35).

 

Be sure to push the "Submit Form" button, after which you should see a confirmation screen.  We'll e-mail an invoice to you as soon as we ship the order.  We're a non-profit, so prompt payment is appreciated or at least within 30 days.  We really do what to serve you with excellence, but we're new and learning, so let us know how we can improve.  Thanks again for investing in young leaders!
